Forms

This is a new section of our website, with the focus of moving much of our paperwork online so that it is easier to use. We eventually hope to have all of our forms listed here.

Currently, we only have our Donation Request form uploaded.

Any organizations interested in computers or other equipment should click the link above, download the form. The form is a PDF which you can fill out electronically, and then submit to us via e-mail. There is a button to do this right on the form, but you can also manually attach it to an e-mail message.

Please fill out the form as best as possible. We apologize that it is a bit long, but the more information we have, the easier it is for us to ensure the donation is prompt and goes smoothly. However, don't worry if there is some information or details that you are unsure about, and feel free to contact us with any questions.
